These materials will not be printed, but are available to download here:

Project Protocol – this gives all the details about the Census

Notes on Completing the Census Spreadsheet – this gives help on the technical aspects of the completing the spreadsheet

Dataset Codes – a full list of the codes used in the dataset

Questionnaire – this contains all the questions and has tick boxes for quick and easy completion. It may be useful to print a copy for each patient to be used on the ward or for administration.

Blank Data Capture Spreadsheet - use this blank spreadsheet as a template for importing Patient data into the main Census Spreadsheet.
